Advance Care Directive – A Personal Example

We plan our finances, retirement and wills, but many of us never clearly record what medical treatment we would or would not want if we became unable to make those decisions ourselves.

I prepared my own Advance Care Directive, and after discussing it with friends, several asked if they could see an example.

I have therefore made a de-identified version available here. All personal and identifying information has been removed.
